Home teams (Teams not to be confused with registered Vikings or Mission teams) - Below is information on how to register your team for the upcoming fall season (2010/2011).
Registration process for fall Season (similar to spring):
1.) Walk-in to pick up registration packets & set up an appointment to return completed paperwork.
Now-May 1st (Monday-Thursday, 10:00am-3:00pm, and Friday 10:00am-12pm)
2.) Your paperwork must be complete on the day of your SFPAL appointment.
Incomplete registrations require you to make another appointment.
* Note: It will take 5 – 7 working days to process your team paper work. *

For your players, you will need to collect:
- Player 1601 Form (available at the PAL office) – Please make sure the parents sign both required fields
- SFPAL Waiver of Liability Form – signed in both places
- Birth certificate or other proof of age – Must be government issued
- Picture – Must be a clear 1” x 1.5” picture that shows the head and shoulders only with no hat or sunglasses being worn.
For your coaches you will need:
- Adult 1628 Form (available at the PAL office) – Please make sure to sign and input all required information marked by an asterisk (*)
- SFPAL Coach/Adult Form
- Copy of Coach license – Each Coach must have at least an F license
- Picture – Must be a clear 1” x 1.5” picture that shows the head and shoulders only with no hat or sunglasses being worn
- Criminal Background Clearance – If you have not already been live scanned through SFPAL, please call our office to get background information. This is needed for registration.
If you will be adding players and/or coaches, you will need to make a follow-up appointment 5 to 7 days after, to pick up your new team roster and passes.
