If you are a newly forming team and are not already registered with Mission or Viking League, the following information is what you will need to do to become an SFPAL team.
Volunteers Wanted! Please help us and we'll help you!
If you volunteer with SFPAL in the spring you'll be helping out with painting lines, setting up/breaking down goals or office work. In return we'll give you special privileges that can include, time of games, bye date request and registration priority for next year! We really appreciate your help and hope that you will consider being a volunteer. If you're interested email Joanna.
Please note the following:
Each team is only allowed three transfers per season. Please make sure you do not have more than 3 players that are already registered with Mission or Viking League.
SFPAL does NOT accept whole U-7 teams. If you have U-7's on your team, you MUST have 50% or more U-8's on that team.

SFPAL Spring League Play |
Call SFPAL 415.401.4666 to make an appointment to pick up paperwork (up to January 9, 2009) – Most of these forms are triplicate forms that cannot be printed out. Once you’ve picked up the paperwork, the following is a checklist of all the items you will need to bring back to us.
For Each Coach:
A coach is considered any adult who will be giving direction and wishes to have access to the player/coach area.
- Adult 1628 Form– Please make sure to sign and input all required information marked by an asterisk (*)
- SFPAL Coach/Adult Form
- Copy of Coach license – Each Coach must have at least an F license
- Picture – Must be a clear 1” x 1.5” picture that shows the head and shoulders only with no hat or sunglasses being worn
- Criminal Background Clearance – If you have not already been live scanned through SFPAL, please call our office to get background information. This is needed for registration.
For Each Player:
- Player 1601 Form – Please make sure the parents sign both required fields
- SFPAL Waiver of Liability Form – signed in both places
- Birth certificate or other proof of age – Must be government issued
- Picture – Must be a clear 1” x 1.5” picture that shows the head and shoulders only with no hat or sunglasses being worn.
For The Team:
- One check totaling the amount for each player in your age division please see the chart below. If your parents would like to make a donation, please have them submit a separate check with “Donation” written in the memo portion

Once paperwork is gathered for your team and is completed please call our office to
make an appointment to drop off your completed paperwork. Appointments are Dec 1, 2008 – Jan 23,2009. When your paperwork is completed, it takes SFPAL two weeks to complete registration. |
